The MS Excel version of the form is designed to be completed on a computer (although it can be printed first and filled in neatly by hand) It has help texts associated with certain fields (look for a red triangle in the top right hand corner of applicable fields).

Who Should Attend?

This is an Advanced Course, and is aimed at Health Care Professionals who have a basic knowledge and understanding of diabetes mellitus. It is designed to give an extensive overview of the core principles of modern team diabetes management, so enabling the particpants to undersatnd the condition in sufficient depth, to make a real difference in the lives of people with diabetes.

Pre and Post Course multiple choice knowledge evaluation tests are administered, to allow for an evaluation of the learning experience.

Typically, attendees include doctors, nurses, pharmacists, biokineticists, and other healthcare professionals. Pharmaceutical company employees (sometimes even those without a formal healthcare qualification), have also attended this course. 

Attendence of this course is a mandatory contractual requirement for doctors wishing to operate as a CDE affiliated "Centre of Excellence".

Course Costs

All prices quoted are applicable for 2012, are in SA Rand, and include VAT.

For Medical Doctors and Specialists:

  • Advanced Course Price:                                                                                        R 7 500
  • Early Bird price (Book and pay at least 30 days before commencement of course)           R 6 300

For Other Delegates (including allied healthcare practitioners):

  • Advanced Course Price:                                                                                       R 5 500
  • Early Bird price (Book and pay at least 30 days before commencement of course)           R 4 950

Payments may be made via direct deposit, credit card, or by a deduction from capitation fees due to the practitioner.

How to Book

Download the application form (see document download section at the top of this page). The following points are important:

  1. Dates (bottom left-hand corner) and costs (top right-hand corner) for the Course appear on the Application Form.
  2. Please complete the application form fully and neatly in block capitals, sign it and fax with proof of payment to +27 (0)86 607-9355. Ensure that you provide us with a valid e-mail address to which we can send your Course confirmation and pre-course readings (N.B. We do NOT fax or courier these). Incomplete forms will be rejected.
  3. Places for each Course are limited to a maximum of 32. Early bookings (60-90 Days prior to commencement) are recommended. In view of the fact that the forms were issued so close to the first Course in March, the following concession applies to the March Course only: forms received by 17h00 on Monday 19th March 2012 will qualify for the “Early Bird” discount. However, please bear in mind the next point…
  4. Availability of places on the Course (places allocated on a “First Come ~ First Served” Basis). N.B. WE EXPECT OUR MARCH COURSE TO BE SOLD OUT WITHIN DAYS OF THIS NOTICE
  5. Allocation of a place on a Course of choice is thus subject to ALL of the following:
    - Our receipt of a fully completed 2012 Application form (One per applicant – you may copy / print forms as needed). The attending delegate is asked to read these notes and to complete and submit their Application Form PERSONALLY. We will not be liable for any errors, loss or inconvenience resulting from delegation of this task to someone else or from the non-receipt of all the relevant information contained here;
    - Our receipt of full payment (Remember that if you book and pay the full fee at least 30 days prior to a Course commencing, you qualify for a substantial “Early Bird” discount).;

Certification and Accreditation as a CDE "Centre of Excellence" Provider

Official completion certificates are provided to delegates who achieve a mark of at least 60% in the final Post-Course Knowledge Evaluation.

A distinction certificate is provided to delegates who achieve a mark of at least 80%.

Attendance at this Course, and achieving a pass mark in the final Post-Course Knowledge Evaluation, are pre-requisites for medical doctors wishing to become providers within the CDE "Centres of Excellence" network.

 

Continuing Professional Development (CPD)

The Course offers 34 contact hours. The Wits Health Consortium CPD Office has accredited the Course for 30 CPD points for Medical Practitioners and other Healthcare Practitioners registered with the Health Professions Council of South Africa. CPD certificates will be provided (if applicable).

Advice

Dress Code:

The dress code for the duration of the course is comfortable, smart-casual.

Flight Arrangements:

The Course will run from 08h30 to 17h00 daily. Please ensure that you book your return flights (if applicable) from 18h30 onwards (19h30 onwards for international flights). This will give you enough time to complete the Course and get to the OR Tambo International Airport on time. We would appreciate your assistance in this regard, as it is extremely disruptive to have participants asking to write their final test earlier so that they can catch an early flight. This will not be allowed. If you choose to leave early and miss the final written test, you will forfeit your Official Course completion certificate.

Meals:

Morning tea, lunch and afternoon tea is provided to delegates. Dietary preferences are catered for, including: Kosher, Halaal, and vegetarian. Certified Kosher and Halaal meals do attract a surcharge of R70 per person per day - If you choose either of these options, please add R350 to the payment for your applicable Course fee. If we are notified of any food allergies, these will be also catered for within reason.

Parking:

Parking at the CDE Central Office is limited, so please make sure that you arrive each day in good time.
